New Patients / PreOp



We are always excited to meet new patients. Our goal is to make you feel as comfortable as possible. Please plan to arrive a few minutes early to allow time to complete the new patient paperwork, if you have not previously done so.

Please remember to bring the following to your first appointment:

  • Your dental and medical insurance card
  • Your surgical referral slip
  • Any recent x-rays

The first visit is essentially the collection of data that will lead to a diagnosis and discussion with you about the findings. Our doctors will review the ideal treatment option(s), along with a thorough discussion about benefits, risks and alternatives. We will also explain the procedures involved and the treatment sequence. Please visit the Policies section for explanation of insurance, payments, and consents forms. Below is a link to our financial policy form that must be filled out prior to appointment as well as the registration.

One of the great things about our practice is the convenience of a same day surgery. This is an option available for emergencies and most but not all dental procedures.

If you are a new patient and have made an appointment for a same day surgery, make sure you have a designated driver and/or escort and nothing to eat or drink 6 hours prior to surgery time. The only exception to this fasting rule is if you take any medications regularly. If this is the case, you are to take a few sips of water with your medications 2 hours prior.

These 2 rules are to allow our patients to have the option of having intravenous sedation/general anesthesia with their surgery. If you are on any blood thinners it is imperative that you speak to your prescribing physician about the management and that you fully disclose this information. Please see our health history forms in the registration forms at the bottom of the page.

There are situations where same day surgery cannot be an option. These are for any medical procedures such as biopsies, impacted teeth, and TMJ.
